Preventative Dentistry
We believe that preventative dentistry is the most important service that we offer. We have an excellent dental maintenance plan, carefully designed to treat your problems while they are still minor.

Emergency Treatment
I Dr. Lucy T. White, your family dentist, can be paged any time, day or night, for serious dental problems. Simply call my office phone for paging details.

We stand by ready to be your partner in dental care.

Sincerely yours,

Lucy T. White, D.D.S.

Welcome!

We hope that you will choose us to help you with your dental needs. The following information is to help you understand our treatment philosophy and our office policies.

What to expect on your initial visit:

A. Medical History - For your protection, I must know the condition of your health before beginning treatment.

B. Examination - Any discomfort is relieved at this time. Exams will consist of one or more of the following: (a) thorough oral structure evaluation including cancer detection, gum disease, and x-rays (to view decay and bony support if the teeth), (b) various diagnostic tests and (c) impression models.

C. Treatment Plan - Following the exam, we will recommend proper treatment. We will explain the condition of your dental health, as well as any dental restoration cost.

D. Payment - Financial arrangements are customarily made during the first appointment. We prefer that a parent of a minor be present during your initial consultation. If you have insurance coverage, you should bring necessary forms with you at this time. Some dental insurance plans insist upon pre-authorization, and most do not provide full coverage.

E. Financial Terms -

1. All co-pays are due at the time services are rendered.

2. For major work, you may pay half the total cost when treatment begins and the other half once treatment is complete.

3. If payment-in-full at the time of treatment is not feasible for you, we can work out a signed financial agreement with monthly payment options.

4. If you have insurance, we will estimate the insurance benefit, and bill you for the remaining balance. You are, of course, responsible for the total cost of your care less the actual insurance benefit.

5. Forms of payment accepted are: Cash, Check, Money Order, American Express, Discover, Visa and Mastercard.

Insurance - Most major PPO’s are accepted. We would be happy to check your benefits and to see if you have an in network or out of network plan. We are sorry that our office can not receive payment from HMO plans.

We support the local ILWU (International Longshore Workers Union) PMA.

Dentures - An artificial substitute for natural teeth and adjacent tissues.

Temporomandibular (TMJ) Problems - The connecting hinge mechanism between the base of the skull (temporal bone) and the lower jaw (mandible).

Oral Hygiene Maintenance - Prophylaxis (cleaning), scaling and polishing procedure performed to remove coronal plaque, calculus and stains.

Mouthguard - Device that fits over the teeth to prevent injury to the teeth, mouth or lips. May also refer to a device that prevents tooth grinding or treats temporomandibular (TMJ) disorders.

Orthodontics

For orthodontic treatment a signed financial agreement can be worked out with an initial payment due when treatment begins then monthly payments over the course of treatment.

Dr. White's orthodontic plan can make a real difference in your smile, and help you achieve the smile of your dreams. An attractive smile is an important part of the overall impression a person makes. It radiates confidence and projects a positive image about you. A beautiful smile is a powerful part of looking and feeling your best. Orthodontics also corrects malocclusions (the misalignment of teeth and jaws). The following are some examples of problems that orthodontics can correct:
